It is short and plain, and it does a lot of work. The four pages hold the terms, your contacts, your pet's details and your wishes in an emergency.
The four pages
Page one has the terms and your contact details, with an emergency contact who is not traveling with you. Page two collects the pet's details: name, breed, age, weight, sex, intact or neutered, any bite history, feeding times, activity level, allergies and health conditions. Page three is the photo release. Page four is the medical release. The boarding agreement page has the PDF.
What the terms say, in plain words
You confirm the pet information is accurate. The kennel agrees to use reasonable care. Posted daily rates apply to the whole stay, and check-out is by 11:30 AM. Comfort items are welcome, but the kennel is not responsible if bedding or toys are lost or damaged. A pet not picked up 10 days after the scheduled date without contact is considered abandoned. The kennel may obtain veterinary care at the owner's expense if needed, and California law governs the agreement.
How to get it
Download the fillable PDF, ask for it by email, pick it up at the office, or fill it in at the first drop-off if you allow extra time. Online accounts through the booking system hold your profile and vaccine uploads, but they do not replace the agreement. Read the question page on that point.
Keeping it current
Tell the kennel when your phone number, address, veterinarian or emergency contact changes. The agreement stays in effect until you revoke it in writing. Related entries: medical release, photo release and abandoned-pet clause.
Why a signed form matters
A boarding stay hands your animal to someone else, so both sides need to know the terms. The agreement tells the kennel who you are, who to call and what you want done in a medical emergency. It tells you what the kennel promises: reasonable care, posted rates and a clear pick-up rule. Reading it once before the first visit saves questions at the counter. If a clause is unclear, ask the staff or call 707-678-6600.
The kennel also keeps a copy of your vaccine records with the agreement, so upload them to your account or send them by email when you return the form.
